Utah County Give Kids a Smile

Saturday, February 24, 2018 at the Utah College of Dental Hygiene in Orem they will be holding the annual Utah County Give Kids a Smile (GKAS) dental event. Volunteer dentists and hygienists will be on-hand to provide exams and perform limited procedures for qualifying children, and refer them for follow-up services after the event if needed.

To qualify for this program, parents of children between the ages of 5-12 years old who are also uninsured and from low income families must submit the enclosed Health History and Consent Form no later than February 9th, 2018. If you meet the qualifications, and would like your child to see a dentist at this event, please fill out the enclosed form and return it to your child’s school as soon as possible, but no later than February 9th, 2018. The Utah GKAS event has limited slots available. Return this form as soon as possible to secure your child’s appointment!
